FAMILY PHYSICIANS TAKING NEW PATIENTS

Please note that our clinic uses "Artificial Intelligence" for clinic note transcription. We would appreciate you providing consent to use this during your and are happy to answer any questions that you may have about this innovative approach.


We are presently only taking people who do not have a family physician or whose family physician is retiring.

Please note that due to significant demand new patient request can take up to 6 weeks to process.

We are not taking any reapplications if you previously declined a family physician at our clinic.

PATIENTS ARE REQUIRED TO HAVE A VALID & CURRENT BC HEALTH CARD

Please note that physicians at this clinic do not prescribe narcotic medication to new patients until the appropriateness is determined.

Patients are required to complete this
Comprehensive Medical Questionnaire. Please note that you have to download and save the form on your computer (under the name of the person applying) as it will not save online or you can print the form and complete it.

Completed forms can be e-mailed to
patients@wsmed.ca or faxed to 250 - 768 - 6907 or returned to the clinic in person at 103 - 1135 Stevens Road, West Kelowna, BC.

PLEASE NOTE THAT COMPLETING AND SUBMITTING THE CMQ FORM DOES NOT MAKE YOU A PATIENT AT THIS CLINIC, YOU BECOME A PATIENT AFTER YOU HAVE MET WITH A FAMILY PHYSICIAN WHO AGREES TO TAKE OVER YOUR CARE. DO NOT TRANSFER YOUR MEDICAL RECORDS UNTIL REQUESTED IF YOU ARE ACCEPTED AS A PATIENT.

Transfer of your previous medical records (when required) is your responsibility and may be a pre-requisite for the family physician to fully accept your care.

Please be aware that to facilitate our professional ability to cross cover our patients you are unable switch family physicians within Westside Medical Associates when you have been accepted by one of our family physicians.
