Becoming a patient

We are currently accepting people who do not have a family physician, or whose family physician is retiring.

Family medicine services at this clinic are only for patients registered with us. If you are already a patient here, you do not need to apply — simply book an appointment.

Before you apply

  • You must have a valid and current BC Health Card.
  • We are only taking people who do not have a family physician, or whose family physician is retiring.
  • We are not accepting reapplications from anyone who previously declined a family physician at our clinic.
  • Because of significant demand, a new patient request can take up to six weeks to process.

How to apply

  1. Download the questionnaire to your computer. Download the CMQ form (PDF). Save it first, under the name of the person applying — for example Jane-Smith-CMQ.pdf. If you start typing into it while it is still open in your web browser, your answers will not be saved.
  2. Open the saved file in Preview (Mac), Adobe Acrobat Reader (Windows) or any PDF app, fill it in, and save it again. You can also print it and complete it by hand.
  3. One form per person applying.
  4. Send the completed form back to us using any of the methods below.

Why there is no online form

This questionnaire asks for detailed medical information, so we deliberately do not collect it through a form on this website. You download it, complete it on your own computer, and send it to us directly. Nothing you write on the form passes through this website at any point.

Submitting the form does not make you a patient

You become a patient at this clinic only after you have met with a family physician who agrees to take over your care.

Do not transfer your medical records until you are asked to. Transferring your previous records, when required, is your responsibility and may be a prerequisite for a family physician to fully accept your care.

Things to know

  • Physicians at this clinic do not prescribe narcotic medication to new patients until the appropriateness has been determined.
  • So that our physicians can properly cross-cover each other's patients, you cannot switch family physicians within Westside Medical Associates once you have been accepted by one of them.
  • Each patient has a designated family physician but has access to all the other physicians at the clinic if their own doctor is unavailable.

Registered patients only Family medicine and our weekend walk-in clinics are available only to patients registered with a family physician at Westside Medical Associates. We are unable to see members of the public who are not registered with the clinic.
